It assumes that one symbol error always yields one bit error, which is clearly incorrect. I replaced the incorrect QPSK example with a correct BPSK example and provided a source. The reason that I switched from QPSK to BPSK is because it removes the need to specify the mapping from bits to symbol, which would distract from the main point of the example.
